CNA 21 Skills Recertification

If it has been a period of 24 consecutive months that you have not provided nursing or nursing-related services for pay, you will need to recertify.  To recertify, review your skills during a three-hour Illinois Department of Public Health approved evaluation session, and then schedule a three-hour on-site clinical session with a Parkland College instructor at a long-term care facility.
 
Your on-site clinical session will be scheduled with the instructor during the classroom portion.
 
IMPORTANT
You must follow these steps prior to attending your scheduled 21 Skills Recertification class:
1. Go to Nurse Assistant website: http://www.parkland.edu/healthprofessions/nurseasst.aspx
-Complete Health CareWorker Background Check (Authorization Form)
-Submit to: Rachel Delaney, 1309 N. Mattis Champaign, IL 61821, 217-353-2760
2. Pick up Livescan Request from Health Professions Office at 1309 N. Mattis
-This is received after the Health Care Worker Background Check is turned in and processed
-This entry may or may not be able to be done at the time you turn in your Background Check, and may require you to return to obtain your Live Scan Request Form.
3. Complete fingerprints at Accurate Biometrics
- Accurate Biometrics, Huntington Towers 201 West Springfield Street, Suite 207, Champaign, Illinois 61820
-Monday and Thursdays, 9-12:30PM and 1-5pm are the best times to go to Accurate Biometrics.
- Do not throw away your proof of fingerprints
4. Bring proof of completed fingerprints to your scheduled class.
